We are currently an "in-network" provider for only Anthem Blue Cross PPO, Blue Shield of California PPO, Cigna PPO, United PPO, Monarch Optum HealthCare HMO, Hoag HMO, and Mission Hospital Allied Physicians HMO.
For patients with PPO insurance, no authorization is needed. For those with HMO insurance, an authorization from your primary care provider is required before scheduling an appointment.
Your insurance company requires us to collect co-payments, coinsurance, and/or deductible amounts at the time of service. We are obligated to do so to remain in compliance with insurance regulations.
Any remaining balance after your insurance has processed your claim will be billed to you. If your insurance plan changes, please notify us before your next visit so we can update your records and help you maximize your benefits.
Our office will handle insurance billing, as well as any required test or follow-up authorizations.
We do not bill insurance directly. Payment is required at the time of your appointment. You will receive a superbill and receipt, which you may submit to your insurance company for potential reimbursement.
Most patients with PPO plans can expect to be reimbursed for 60% to 70% of the visit cost. We recommend contacting your insurance provider before your appointment to learn more about your out-of-network benefits, deductibles, and any coverage limitations.
Initial consultations are typically 60 minutes long, with a fee of $500. Follow-up appointments are 30 minutes, with a fee of $200.

Dr. Przeklasa Auth, or a covering pediatric neurologist, is available 24 hours a day for urgent medical concerns. For emergencies, please call 911 or go to the nearest emergency room.
Dr. Przeklasa Auth will prescribe enough medication to last until your next recommended follow-up appointment. If a refill is needed before that visit, please submit your request directly by email to our office at occhildneuro@gmail.com, rather than through your pharmacy.
To help ensure accurate and timely processing, we kindly ask that you allow up to 72 hours for all prescription refill requests. We also recommend planning ahead to avoid interruptions, especially around weekends or holidays.
Please bring your forms to us as soon as you receive them. Before submitting, make sure to complete all parent sections and clearly write your child’s name and date of birth.
If you bring a form to your appointment, we will do our best to complete it before you leave. However, during busy times, you may be asked to leave the form, and it may take a few days to process.
If you are dropping off, faxing, or emailing a form, please allow 3 to 5 business days for completion. We will call you when your form is ready for pickup.
Please note: there is a $25 form completion fee.
